Payment Information

New Patient Appointments:

(60-120 minutes—in person) $200-$400


Follow up Appointments:

(60 minutes - In person or telehealth depending on the need) $150


15-30 minute Follow Ups:

(In Person or Telehealth depending on the need) $75


School/Sports Physicals:

(30 minute visit, ages 16 and up, in person only) $75


Other Focused Visits—$50-$100

(Paperwork, forms, new lab orders, medication reconciliation, referrals, etc. 15-60 minutes depending on the specific situation)

  • All major credit/debit cards
  • Cards will have a $5 convenience fee added.
  • Venmo
